Clinical performance evaluation of one to one interaction simulation program using standardized patient simulation for nursing students

Abstract
Background: A one to one interaction simulation using standardized patient (SP) is needed for a successful clinical performance (CP) evaluation. The purpose of this study was to investigate nursing students¡¯ CP score evaluated by instructor, student, and SP.

Methods: A one to one interaction simulation using SP caring for patients with oxygenation needs was designed and applied to 76 junior nursing students approaching their first clinical practicum. After simulation, nursing students¡¯ CP score evaluated, and experiences of SP and students during debriefing process also analyzed using video watching to examine what the students learned.

Results: Total score of students¡¯ CP evaluated by instructor was higher than students¡¯ self evaluated CP. Also total score of students¡¯ CP evaluated by instructor correlated with students¡¯ self evaluated CP score. Especially it is highly correlated with students¡¯ CP score in intervention domain between instructor¡¯s and student¡¯s evaluation. SP¡¯s evaluations of students¡¯ strength were empathy and caring, teaching and explanation. Lack of confidence, lack of empathy and caring were student¡¯s weakness of SP¡¯s evaluation. Over one third student experienced find enlightenment as feel confident never make a mistake. Students¡¯ accomplishment and frustration were dependent upon patients¡¯ condition.

Conclusion: These results suggested that one to one interaction simulation using SP program would be an effective strategy to enhancing student¡¯s confidence level.
